What this covers

This is about a young woman called Hannah who had been sexually groomed by her manager in her place of work. Eventually, this ended up with her being pimped for sex by her boss. She was overwhelmed by shame.

The therapist had used the standard protocol to desensitize several target memories but there were aspects of the trauma that were too shameful for the client to verbalise.

The video demonstrates the use of the Blind to Therapist protocol so that the client could desensitize and reprocess the impact of those particular parts of the trauma without having to describe them aloud.

  1. Intervention during the desensitization and reprocessing phase of the protocol.
  2. The need to use Blind to Therapist (B2T) identified during the assessment / history-taking phase when the client couldn’t disclose information about the target memory
  3. Clients who can’t or won’t disclose may come in different categories:
    • Shame and embarrassment
    • Fear of vicariously traumatising the therapist or a translator
    • Fear of repercussions from translators who come from the home community
    • Clients with communication problems such as severe stammers
    • Those working under the Official Secrets Act who are forbidden by law to disclose
  4. Explain that treatment using B2T will not be less effective due to non-disclosure
  5. Coach the client to recognise change as the therapist may have difficulty identifying whether or not change is happening.
  6. Give undisclosed image a cue word
  7. Do not obtain NC or PC
  8. Get a SUDs score
  9. Start the first set by saying ‘Notice (cue word), (emotion), (body location)
  10. Process as normal until SUDs reach 0 or 1
  11. Disclosure may never occur but PC may emerge
  12. Even with PC don’t attempt to obtain a NC
  13. Install PC if one surfaces in the normal way
  14. VOC PC to 6 or 7
  15. If SUDs 0 and no PC arises or if a PC has been installed do a body scan with target image and PC in the usual way

Use of Visual Interweaves

Recognise that cognitive interweaves are impossible because the therapist has no cognitive knowledge to use in creating a suitable interweave. Instead, therapists use visual interweaves. These take the form of either image strategies or morphing strategies.

Image Strategy

In an image strategy, the client might be asked to imagine the stuck image at arm’s length and on the left. Then they’re asked to imagine the same image at arm’s length away on the right.

The client is then asked to look at both images and say in which of the two do they feel most in control of what’s happening. When the client tells the therapist which image is more in his or her control, they are asked to bring the two images together until they just touch each other and bilateral stimulation is applied.

This image strategy will often release processing that had been stuck.

Morphing Strategy

Another form of visual interweave is morphing. Here the client’s asked to look at the stuck image and imagine how he or she can stretch the whole picture so it looks funny in some way. Once the client has the new image, bilateral stimulation is applied.

These techniques will often unblock stuck processing.
