Special note

Always use DES (Dissociation Experience Scale) to help distinguish between common dissociation and Dissociative disorders, a much more serious condition. In this case, the client’s score of 35 is indicative of a Dissociative disorder, the likely existence of more than one ego state, i.e.of ‘parts’ of the personality, and therefore the need for the therapist to proceed with caution.

What this covers

Case of woman with history of childhood physical and emotional abuse and showing signs of Secondry structural dissociation with several parts to her personality.  Video shows how to work safely when there is a high risk of dissociation, which can lead a client to lose contact with the present moment and be caught up in the past when asked to describe or focus on a traumatic event.  The client may become dys-regulated and re-traumatised.  In desensitisation, dual attention – one foot in the present and one in the past - may be lost and the process could stall.

  1. Explain CIPOS and BHS to client and why it might be useful. Explain procedure step by step.
  2. Get client’s permission to use (and from any ego states, if present)
  3. Arrange to allow for a 90 minute session to use this intervention, if possible.
  4. Ensure client is aware of the objective reality of being in therapy room. Strengthen present orientation by asking client questions that involve them using any of their five senses and strengthen with short (5-8 sweeps) of slow BLS (EM’s or other stimulation methods you might be using). Emphasise distance – geographic and time – between present situation and the traumatic event to further strengthen present orientation if needed. As a backup, keep a cushion handy to play ‘catch’ if you need to quickly bring the client back to present reality.
  5. When present orientation is established, begin having the client close their eyes and access a memory image for a very brief period – 2 seconds – while you keeping track of time.
  6. Immediately after the end of the agreed time, using soothing tones but with an empathetic voice, say, ‘Come back into the room now’ (repeat the command if necessary until the client’s eyes are open and looking into the room. Give encouragement, ‘Good’ or ‘You’re doing well’. Then ask ‘Where are you right now on the line? And follow this with a short set of slow EM’s).
  7. Repeat this process gradually increasing time spent ‘in the memory’ until the client is able to do this for up to 20 seconds and come out and be at least 3 inches in front of their face. When this has been achieved, they are more ready to do the full standard protocol on the target memory.
  8. Finally, Knipe recommends that if a client is unable to tolerate as little as 2 seconds ‘in the memory’, do not use CIPOS without doing further orientation to the present and until the 2 second immersion is satisfactorily achieved. Alternatively try Flash, Video 006.
